Instructions to Load Images

What are the step-by-step instructions for sharing lab results, pathology reports, and clinical notes?

1. Gather the following documents:
   1. Recent PSA level(s)
   2. Prostate pathology report
   3. Recent clinical notes by Urologist or Primary Care Physician
2. Once these items are in your possession, follow the steps below to continue uploading:
   1. Click the link New Patient Information.
   2. Scroll down to the bottom of the page, in area titled “documents needed” and click on the light blue tab that says, “Add Files.”
   3. Click on “Choose Files”
3. Choose your file to upload from location that it was saved in electronic format “file name”, then click “open”
4. Click “Upload”
